That calculator isn't an order form. It just tells you how much you'd pay for 50. It asks how much you pay now and your state, and calculates what your savings would be. If you want to know how much a hundred would be just double the price given.
I don't know what the qualifications are for the $15 a month advertised. You can call this number:

(7a.m. – 9 p.m. M–F Central Standard Time) and ask them any questions. There would be too many variables for me to check out every one. I basically get you to the right screen (there are many before this one) and generally check out that the place is legit. The rest is up to you.

Was all excited about the maximum $15 copay on the Accuchek Aviva, so, I went on their website to order the discount card, since I already have the meter. Here's the result:
"Unfortunately, Massachusetts state law does not allow us to send the ACCU-CHEK Connect prescription discount card to Massachusetts residents with primary insurance coverage."
Oh well. Can't win 'em all.
